The Rakai Health Sciences Program (RHSP) is an acclaimed biomedical research and service delivery organization in Uganda. It was founded in 1987 as the “Rakai Project” following the first clinical identification of AIDS in the Rakai District by Ugandan physicians in 1982. Operating as a collaborative venture between Makerere University, Johns Hopkins University, Columbia University, and the U.S. National Institutes of Health (NIH), the organization has evolved into a premier epidemiological research center in Africa. 
The cornerstone of the institution’s global scientific contribution is the Rakai Community Cohort Study (RCCS), launched in 1994. This pioneering, population-based cohort study tracks thousands of individuals across dozens of continuously surveyed communities and fishing villages in south-central Uganda. Data harvested through the RCCS has fundamentally shaped global HIV/AIDS policies and international health strategies. Crucially, RHSP’s landmark randomized clinical trials provided the definitive scientific proof that voluntary medical male circumcision (VMMC) reduces female-to-male HIV transmission by over 50%, a breakthrough that revolutionized global HIV prevention protocols.
Beyond its foundational academic research, Rakai Health Sciences Program is a vital implementation partner for large-scale healthcare delivery across the greater Masaka region. Supported heavily by funding from the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) and CDC, RHSP manages extensive District-Led Programming. This incorporates comprehensive anti-retroviral therapy (ART) administration, prevention of mother-to-child transmission (PMTCT), and the DREAMS initiative, which focuses on keeping adolescent girls and young women safe, mentored, and AIDS-free. The program integrates these infectious disease interventions alongside mobile health clinics and targeted management of sexually transmitted infections (STIs). 
Moving forward under its modern strategic blueprint, RHSP is expanding its scientific footprint to address the shifting public health landscape. Modern initiatives explore the nexus of infectious conditions and non-communicable illnesses, such as the Vicinity Study, which tracks cardiopulmonary diseases among rural-to-urban African migrants. Simultaneously, the organization has modernized its physical operations; in 2025, it completed a major green transition, converting its expansive research and laboratory campus in Kalisizo to net-zero solar power. Through its cutting-edge infrastructure, RHSP continues to host global training frameworks, such as the Fogarty ACHIEVE Fellowship, cultivating the next generation of public health leaders
