Job Description

The Home Care Coordinator is responsible for identifying, coordinating, and growing the hospital’s home care services by ensuring a seamless transition of patients from hospital to home. The role works closely with consultants, physicians, geriatricians, intensivists, nurses, case managers, therapists, and other multidisciplinary team members to identify patients who would benefit from continued care at home and facilitate timely referrals. The coordinator serves as the primary link between inpatient services, patients and families, and the home care team, ensuring continuity of care, improved patient outcomes, reduced readmissions, and an exceptional patient experience. The role is also responsible for developing the home care business by increasing referrals, strengthening relationships with clinical teams, educating patients and families on available home care services, and monitoring service quality and patient satisfaction. This position combines clinical coordination, patient advocacy, business development, and operational oversight to ensure that home care services become an integral extension of the hospital’s continuum of care
